The Mount Group Practice accepts new patients from the following postcode areas: DN1, DN2, DN3 and DN4.
To register with the Practice please attend reception at 54 Thorne Road or Bessacarr Medical Centre or telephone reception. The receptionist will ask you to complete a registration form and a health questionnaire.
To save time you can complete an online registration form.
You can also download a form to complete at home..
You will need to complete this by hand and bring it to the surgery or post it to us..
You will be offered a New Patient Health Check with the Healthcare Assistant, which will include height, weight, alcohol and smoking status, any known allergies, past medical history and any previous family medical history. Following registration the Practice will automatically apply for your medical records to be transferred in.

Alternatively, if you move away from DN1 – DN4 to re-register with a new practice of your choice, simply approach them and the registration process is likely to be as above or similar. Your medical records will then be transferred automatically.
